If you have a friend or relative that you’d like to refer, just give us a call. We’ll be happy to mail you out some referral cards.

Please just put your name, and your friend’s name, on the card and have them present it when they come to clinic. A member of our staff will sign and date the card and put a copy in your file so that you’ll receive the proper credit on your next renewal visit.
